Nous sommes à la recherche d’un(e) coordonnateur(trice) bilingue des soins aux patients (poste mobile) afin de soutenir les cliniques INVIVA à travers le Canada. Dans ce rôle, vous serez responsable de coordonner les services aux patients, de gérer les références et d’assurer une prestation de soins fluide, conformément aux niveaux de service établis.
En tant que point de contact clé entre les patients, les professionnels de la santé et les équipes internes, vous jouerez un rôle essentiel dans l’offre d’une expérience patient exceptionnelle dans un environnement de soins de santé dynamique. Ce rôle exige de la flexibilité, puisque vous interviendrez comme ressource mobile pour soutenir divers programmes et équipes au besoin.
Responsabilités principales :
- Traiter les nouvelles références de patients, les renouvellements et les demandes de service dans les délais établis
- Coordonner les services aux patients, incluant la planification, l’accueil et les suivis
- Examiner et traiter les rapports post-visite (PVR) et les événements indésirables (EI) de manière précise et en temps opportun
- Communiquer avec les patients et les professionnels de la santé en français et en anglais
- Maintenir une documentation exacte dans les systèmes internes conformément aux normes de conformité
- Gérer les appels entrants/sortants, les courriels et les demandes de service selon les indicateurs de performance (KPI)
- Collaborer avec des équipes multidisciplinaires (infirmier(ère)s, gestionnaires de cas, pharmacies et parties prenantes)
- Soutenir la coordination des rendez-vous, y compris les replanifications et suivis des absences
- Assurer une progression efficace des dossiers patients dans leur parcours de soins
- Participer à une rotation de garde et à des quarts de travail prolongés, selon les besoins de l’entreprise
Exigences de base :
- Maîtrise du français et de l’anglais (à l’oral et à l’écrit) – obligatoire
- Minimum de 3 ans d’expérience pertinente en services aux patients, coordination en santé, centre d’appels ou soutien administratif
- Expérience démontrée dans un environnement à volume élevé (coordination, planification, service client)
- Capacité à gérer plusieurs priorités et à travailler de façon autonome avec peu de supervision
- Bonne maîtrise des outils informatiques (ex. : MS Office, CRM ou systèmes de planification)
Compétences et atouts :
- Expérience dans les programmes de soutien aux patients, pharmacies spécialisées, cliniques de perfusion ou navigation en santé
- Connaissance de la terminologie médicale, des processus de remboursement ou des flux de travail cliniques
- Expérience dans le traitement des références, de la documentation d’assurance ou de la coordination de dossiers
- Souci du détail, sens de l’organisation et excellentes aptitudes en résolution de problèmes
- Capacité à évoluer dans un environnement rapide axé sur la performance
- Expérience dans un rôle mobile ou soutien de plusieurs programmes/équipes

About the Role:
We are seeking a Bilingual Patient Care Coordinator (Floater) to support INVIVA Clinics across Canada. In this role, you will coordinate patient services, manage referrals, and ensure seamless delivery of care within established service levels.
As a key point of contact between patients, healthcare providers, and internal teams, you will play a vital role in delivering an exceptional patient experience in a fast-paced healthcare environment. This role requires flexibility as a floater, providing coverage across multiple programs and teams as needed.
What You’ll Do:
- Process new patient referrals, renewals, and service requests within established timelines.
- Coordinate patient services including scheduling, intake, and follow-ups.
- Review and process Post Visit Reports (PVRs) and Adverse Events (AEs) accurately and promptly.
- Communicate with patients and healthcare providers in both English and French.
- Maintain accurate documentation in internal systems in accordance with compliance standards.
- Manage inbound/outbound calls, emails, and service inquiries within KPI expectations.
- Collaborate cross-functionally with nurses, case managers, pharmacies, and program stakeholders.
- Support appointment coordination, including rescheduling and no-show follow-ups.
- Ensure patient cases move efficiently through the care pathway.
- May participate in on-call rotation and extended shift coverage, based on business needs.
Basic Requirements:
- Fluency in both French and English (written and verbal) – required.
- Minimum 3+ years of related experience in patient services, healthcare coordination, call center, or administrative support.
- Proven experience handling high-volume coordination, scheduling, or patient-facing support.
- Demonstrated ability to manage multiple priorities and work independently with minimal supervision.
- Strong computer proficiency (e.g., MS Office, CRM or scheduling systems).
Preferred Skills / Experience:
- Experience in patient support programs, specialty pharmacy, infusion clinics, or healthcare navigation.
- Knowledge of medical terminology, reimbursement processes, or clinical workflows.
- Experience processing referrals, insurance documentation, or case coordination.
- Strong attention to detail, organization, and problem-solving skills.
- Ability to thrive in a fast-paced, KPI-driven environment.
- Experience supporting multiple programs or working as a floater across teams.
Work Environment & Other Requirements:
- Hybrid or remote role with required in-office presence once per month.
