About the Role

We'reseeking aBilingual Field Nurse Case Manager (RN)to support patients, healthcare professionals, and clinic partners through a Patient Support Program (PSP) focused on specialty and biologic therapies.

In this field-based role,you'llserve as a key clinical resource and primary point of contact throughout the patient journey, helping patients access therapy,understandtheir treatment, and remain engaged in care.You'llcollaborate closely with physicians, clinic teams, patients, pharmacies, and internal stakeholders to deliver an exceptional patient experience while supporting clinics across the Greater Toronto Area (GTA).

This role requires regular travel to clinics and healthcare sites throughout the GTA.

What You'll Do

  • Serve as the primary clinical contact for patients, physicians, and clinic staff enrolled in the Patient Support Program.
  • Support patient onboarding andfacilitatetimelyaccess to prescribed specialty medications.
  • Deliver patient education on disease states, treatment expectations, medication administration, side effect management, and adherence strategies.
  • Conduct injection and self-administration training inclinic, virtually, or in-home asrequired.
  • Collaborate with healthcare providers to coordinate patient care and communicate relevant updates.
  • Assistwith reimbursement and patient access activities, including gathering required documentation and supporting patient enrollment processes.
  • Conduct patient chart reviews and collect information required for reimbursement or clinical program activities.
  • Monitor patient progress and support adherence throughout the treatment journey.
  • Identifyand escalate issues requiringadditionalclinical or operational support.
  • Build andmaintaintrusted relationships with physicians, clinic staff, pharmacies, and patients.
  • Document patient interactions and program activities accurately andin accordance withprogram requirements.
  • Support ongoing program initiatives and continuous improvement activities.

Basic Requirements

  • Active and unrestrictedRegistered Nurse (RN) license in Ontario.
  • Fluency in bothFrench and English (written and verbal).
  • Previousexperience working within aPatient Support Program (PSP).
  • Valid Ontario driver's license and access to a reliable vehicle.
  • Ability and willingness to travel up to60% of the time (approximately 3 days per week)throughout the GTA.
  • Strong patient education and communication skills.
  • Proficiencywith Microsoft Office and digital documentation systems.

Preferred Skills & Experience

  • Additionalactive nursing licenses in other Canadian provinces.
  • Experience supporting specialty pharmacy, biologic, immunology, rheumatology, dermatology, gastroenterology, or other specialty therapeutic areas.
  • Experience providing injection training and patient education.
  • Knowledge of reimbursement, patient access, or specialty medication programs.
  • Experience working directly with physicians' offices, infusion clinics, or specialty clinics.
  • Familiarity with biologic therapies and pre-treatment requirements, including laboratory testing, diagnostic imaging, and screening protocols.
  • Strong relationship-building and stakeholder management skills.
  • Experience working independently in a field-based healthcare role.

Travel, Work Environment & Physical Requirements

  • Field-based position supporting clinics across the Greater Toronto Area (GTA).
  • Travel required approximately60% of the time (3 days per week).
  • Mileage and approved travel expenses are reimbursedin accordance withcompany policy.
  • Remaining time may be spent working remotely from a secure home office environment.
  • Must be able to travel between clinics, physician offices, and patient locations asrequired.

The pay range shown below is aligned with McKesson's pay philosophy, and pay will always be compliant with any applicable regulations. $56,800 - $75,700
